Abdominal distension and tenderness

A 45-year-old man presented with abdominal distension, tenderness, discomfort and erythema of the abdominal wall in the midline of the abdomen. Prior medical history included Fournier's gangrene complicated with enterocutaneous fistula and midline abdominal hernia for which he had undergone surgical debridement of the infected tissues and abdominal wall reconstruction using two synthetic meshes two years before. The CT scan (figure 1) of the abdomen showed inflammation in the abdominal wall.
